NEW DIAA REQUIREMENTS

2026-2027 Required Medical Forms

All health information required within the RankOne and Magnus Health accounts for each child attending Archmere must be submitted no later than August 1st. If your student plans to participate in a fall sport, the student will not be eligible to practice until the forms are processed and reviewed.

  2. DIAA Acknowledgement Form

  3. Pre-Participation Consent Form 

  4. Emergency Contact Card

  5. Medical Card Upload Form – Upload a copy of the Student Athlete Medical Card, completed and signed by a physician.

DIAA Tournaments - Online Ticketing

All tickets for DIAA post-season tournament events will be done online using  GoFan - DIAA  Tickets will be validated on your cell phone at the entrance to the event.  No pets!
NO CASH TICKETS WILL BE SOLD AT THE DIAA GAME SITES!!
